to hire per diem Personal Care Associates to service Franklin

Driver's License and Current Auto InsuranceMust be at least 18
years of ageWeekend and holiday rotation requiredContinued
education/training:Earn your LNA for free - Eligible employees that
have been with the VNAHSR for one year and worked a minimum of 1000
hours can have their LNA course and materials completely paid for,
and will receive a raise upon receipt of
certification.Responsibilities include but are not limited
to:Collects patient and family data and reports findings to
supervisor in a timely manner.Assists in the communication of
information to assigned patients.Under the supervision of the
Staffing Manager, performs interventions and procedures as directed
to ensure the safe effective quality of care for patients.Reports
to supervisor on a variety of aspects of patient and family
information including, but not limited to identified problems, care
provided and patient's responses to meet patient and family needs
and ensure overall quality of care delivered.Maintains up-to-date
and accurate documentation of care provided to ensure the effective
integration of information for use by the health care team to
ensure on-going and continued quality of care.Advocates for patient
care issues to ensure that overall quality and type of care is
sensitive to each specific patient/family's needs.Carries out a
variety of household chores, including but not limited to sweeping,
mopping, laundry, doing dishes, cleaning bathrooms, vacuuming,
simple meal preparation and serving, etc. Ensures that tasks are
performed to patient satisfaction.Stays up to date on Homemaker
"Do's and Don'ts" requirements.Proactively reports changes in skin
conditions to RN, Case Manager or program manager in a timely
manner.
