Job Description:
A company is looking for a Healthy Planet/Compass Rose
Consultant to lead workflow discovery and develop training programs
for care management and population health.
Key Responsibilities
Conduct workflow discovery and document existing processes related
to care management and population health
Collaborate with stakeholders to identify training needs and
develop comprehensive training documentation
Facilitate training sessions to ensure effective knowledge transfer
to new staff
Required Qualifications
Proven experience with Healthy Planet and Compass Rose in care
management and population health workflows
Strong expertise in workflow discovery and training development
Experience in developing and delivering training programs for
diverse audiences
Familiarity with outreach and care management for specific patient
populations
Ability to work independently and meet tight deadlines
