Job Description:
A company is looking for a VP of Value-Based Care
Operations.
Key Responsibilities
Lead the implementation and oversight of the CMS GUIDE Model and
ensure compliance with quality benchmarks
Develop strategic relationships with ACOs to position the
organization as a preferred provider for post-acute care
services
Oversee training and performance management for Care Navigators to
ensure high-quality care coordination
Required Qualifications
Bachelor's or Master's degree in Nursing, Social Work, Public
Health, Healthcare Administration, or a related clinical field
Preferred advanced certifications in dementia care management or
value-based care program leadership
5+ years of clinical leadership experience in dementia care or
post-acute care management
Experience in developing ACO partnerships and value-based care
strategies
Deep understanding of CMS models and value-based reimbursement
frameworks
