Delivering Care with Autonomy and FlexibilityThe University of Vermont Health Network is seeking a skilled and compassionate registered nurse to join our Adult Home Care team. Our Community Health Nurses provide high-quality, patient-centered care in the comfort of patients' homes, working to the top of their licensure. As part of our comprehensive team, you'll collaborate with RNs, PTs, LPNs, LNAs, OTs, SLPs, and social workers to support patients' goals and preferences.Key Responsibilities

• Deliver skilled nursing care to patients in their homes, prioritizing their needs and preferences
• Partner with patients, families, and interdisciplinary teams to develop and implement care plans
• Maintain accurate and comprehensive patient records, adhering to regulatory requirements
• Stay current with industry developments and best practices, sharing knowledge with colleaguesRequirements
  • Greater than two years of home or public health nursing experience
  • Strong communication and interpersonal skills, with ability to work collaboratively within teams
  • Ability to remain flexible and adapt to changing patient needs and prioritiesIncentives
    • $10,000 Sign-On Bonus for external applicants (prorated for part-time roles)
    • Mileage reimbursed at Federal Gas Rates
